F-1 Counselor Forms

Students are expected to bring these forms with them to any meeting or provide them via email when working with an OCC Counselor on any F-1 requests listed below. If Counselors have any questions regarding the form or the student's request, please connect with ISS via Teams or email.

Financial Requirements for F-1/M-1 

The documents below outline the F-1/M-1 student visa financial requirements for students. Document submissions not meeting these requirements will be denied.

Affidavit of Financial Support	Students complete this form as part of their financial requirements

 

Curricular Practical Training Forms

The documents below are required for F-1 students seeking Curricular Practical Training. These should be completed by the student and the internship coordinator for the program of study/internship course. In some cases, Career Services may complete these forms with the student.
